Description

Lymphoblastic lymphoma, thymic T-cell origin and marginal zone lymphoma, spleen

Notes

Lymphoblastic lymphoma, thymic T-cell origin and marginal zone lymphoma, spleen. The thymic mass in the mediastinum and splenic red and white pulp (PALS) contain thymic T-cell lymphoma cells. The tumor cells have round nuclei and prominent nuclei and have metastasized to the PALS and red pulp of the spleen. The starry sky effect (tingible-body macrophages) can be seen in both tissues. There is invasion of cardiac muscle and in peribronchial and perivascular locations in the lung. The splenic marginal zone is expanded in size and contains a population of large marginal zone B-cells with a roundish nucleus and a small nucleolus and pale eosinophilic cytoplasm. This lesion is an early marginal zone lymphoma of splenic origin. The primary thymic lymphoma and metastatic tumor cells in spleen are CD3+ and Tdt+. Some of the marginal zone lymphoma cells are CD45R+ but CD3- and Tdt-.
